I use SpamAssassin, which works wonders. I use it in the form of Eudora plugin, which also exists for OSX. I'm pretty sure I remember the existence of a standalone OSX version as well.
I run a national internet provider. Each of the options that have been recommeded are good, but totally depend on the operating system. So, I'll go with another OS preference (Linux) run qMail with rblsmtp (with assorted spam agents, such as spamcop spamhaus, etc), also apply Russ Nelson's executable patch, and finally run qscanq passing emails through clamd. I get no spam/email virii,s same things with my clients (same thing with commercial clients).
